Sunshine Visiting Physicians is pleased to announce a collaborative agreement with Hospice By The Sea, located in Boca Raton Florida. Hospice By The Sea is a Florida licensed, Medicare certified and Joint Commission certified hospice program providing hospice services in Palm Beach and Broward counties.

Collaboration between our organizations provides greater continuity of care for patients Sunshine physicians care for at facilities throughout the Tri-county area, that are referred to Hospice By The Sea, for end of life services.  Additionally, Hospice By The Sea desires greater exposure in facilities where Sunshine Visiting Physicians have a presence.

This is a win-win for hospice eligible clients in our community and we are happy to be involved with an organization well known for providing first class care to the frailest among us.
